Camp Information for Parents
Welcome to the Skills Ontario Summer Camp Program - thank you for registering your child!
The following forms must be reviewed and completed prior to your child arriving at camp.
MUST BE COMPLETED: Informed Consent Form
Please complete this Informed Consent Form by noon on the Thursday before camp at the latest. This signed form is required for your child to participate in this program. Please be sure to indicate your optional preference for photo release and independent sign-in/out.
Some camps require an additional consent form from the host location. If this is the case, you will find a link to the form in the Camp Location Details table below. Please print this form, sign and bring in on the first day of camp, as they cannot be submitted online.
If the online Informed Consent Form is not completed by noon on the Thursday before camp, you must print a copy (linked HERE) and bring in a physical copy on the first day of camp.
Please read this letter for general details on what to expect at the week of camp.
